Membership runs from January through December.
Annual dues are $40. AMA membership is required.

Please complete the Membership Application Email Form below or download, complete, and print the pdf version of the Membership Application to mail or bring to the meeting. NOTE: Apparently the email feature below does not work with Mail for Windows 10. Mail for Windows 10 users must manually copy the required data into an email and send (See bottom of page).
